放射科當偵探 沈彥君銳眼揪病因

如果說臨床醫生都是透過問診來判斷病患病情的先遣部隊，那麼從早到晚坐在暗黑小房間內判讀上萬張X光片的放射科醫師，就像是從照片中找到破案線索的偵探，當醫師怎麼也找不出病因時，放射科醫師的銳眼往往成為最後的破案關鍵。
今年37歲，有著漂亮的外表、說話帶有娃娃音的北投健康管理醫院主任沈彥君，從小由媽媽獨力扶養三個孩子長大，她說當初會選放射科，是因為念醫學系時，媽媽生病擔心孩子掛慮，竟然都沒說，最後檢查才發現腦部腫瘤已經七、八公分，自己卻甚麼也沒幫上忙。最親愛的媽媽，來不及看到自己成為醫生的模樣，她一直覺得遺憾，卻也種下她致力健檢和預防醫學的因子。
一般民眾以為放射科醫生只會拍照，事實上拍照是放射師的工作，放射科醫師則負責判讀，除心臟導管外，病患全身上下所有部位都要會看、會放導管，包含肝腫瘤栓塞、子宮腫瘤等，要會找到「通路」，放管子進去，如果沒有外科手術的巧手，也難以勝任。
她說，許多患者到院時常死氣沉沉，但醫師怎麼也找不到原因，曾有一名70多歲的伯伯只不停喊肚子痛，照了各式片子，體重仍直直落，醫生卻說他沒問題，後來沈彥君幫他做腹部磁振造影，才發現肚子裡有整串的淋巴結；還有坐著輪椅的奶奶全身無力，也說不出哪裡不舒服，醫生和家人都以為奶奶中風，結果是腦部長腫瘤且已轉移到肺部。
「發現病因一方面有成就感，但另一方面，往往也表示快來不及了…」，沈彥君坦言，放射科醫師的工作成就感來自於找到病因，但是找到後能做的卻很有限，因此後來她離開榮總改到健康管理醫院從事預防醫學工作，協助民眾透過健檢來預防各種疾病。
沈彥君建議，許多人都認為40歲以後再幫自己安排常規性健檢，不過近年癌症患者有年輕化趨勢，過了30歲後就可以幫自己安排低劑量的電腦斷層檢查，尤其是有家族病史、糖尿病、高血壓患者等都是高風險者，都應定期追蹤檢查。
